Retired Employees’ Group Life Insurance Program
Introduction
The R.R. Donnelley & Sons Company (“RR Donnelley”) Retired Employees’ Group Life Insurance Program (“Retired Employees’ Group Life Insurance”) pays benefits to your beneficiaries in the event of your death.
As long as you are eligible, the Program offers the following coverages:
 | Company-Paid Coverage
|
 | Basic retiree life insurance
|
 | Additional Coverages
|
 | Optional retiree life and accidental death and dismemberment insurance (available only if you’re a grandfathered Moore Wallace retiree who retired between
January 1, 1979 and December 31, 2001)
|
 | Optional retiree life insurance |
If you’re eligible for additional coverages, you pay the cost of these coverages. Be sure to read all of the benefits information available for these additional coverages, and ask questions so that you can make coverage decisions that are best for you and your family.
This information, along with the certificates of coverage, is your Summary Plan Description (SPD) and explains your life insurance coverage as of January 1, 2005 (unless noted otherwise). The information in the SPD is intended to serve as a summary of the Life Insurance Program. Because insurance coverage is provided through a group insurance contract with the claims administrator, the claims administrator has developed certificates of coverage to describe the coverage available to you under the Program. These certificates of coverage are included with this content.
This SPD, along with the certificates of coverage, explain your basic retiree life insurance. It details who is eligible for coverage, when coverage begins and ends, the benefits provided, how to apply for a benefit, and your rights under the Program. Please read this information to familiarize yourself with the Program.
The Plan document for the Life Insurance Program is entitled the “R.R. Donnelley & Sons Company Retiree Welfare Benefits Plan” (“Retiree Welfare Benefits Plan”). It governs the Life Insurance Program and includes more details on how it operates.
Minnesota Life is the claims administrator for the Life Insurance Program.
You are eligible for coverage under the Program only if you are a member of a participating employer or subsidiary as identified in the certificates of coverage. As you review the certificates of coverage, be sure to check the eligibility to determine if the Program’s coverage applies to you. If you are a retiree of an employer or a subsidiary that does not participate in this Program, you are not eligible for the benefits described in this SPD. If you have a question about the Program, contact the eligibility administrator.
This SPD and any supplemental information, including the certificates of coverage, attempt to be as complete, accurate, and up-to-date a description as possible of your coverage under the Program. However, this summary is based on the Plan document and insurance contracts. If there is any discrepancy between this SPD versus the life insurance contracts, the life insurance contracts always govern. You and your beneficiaries should not rely on any verbal description of the Plan or benefits because the insurance contracts always govern.
In addition, nothing in this SPD should be interpreted as a guarantee of continued coverage. This summary merely describes the coverage offered to retired employees as of January 1, 2005. RR Donnelley reserves the right to change or terminate the Plan or Program, in whole or in part, at any time.
This content contains a summary in English of your rights and benefits under the Program.
