Introduction

The R.R. Donnelley & Sons Company (“RR Donnelley”) Life and Accident Insurance Program (“Life and Accident Insurance”) offers coverages to help you protect yourself, your spouse, and your eligible dependent child(ren) from the financial difficulties brought on by a death or a severe injury.

Your Life and Accident Insurance Program options are listed in your enrollment information. As long as you are eligible, the Program offers the following Company-paid and additional coverages:

Company-paid coverage

Basic employee life insurance

Additional coverages

Employee supplemental life insurance

Spouse life insurance

Child life insurance

Voluntary accidental death and dismemberment insurance

You pay the cost of your additional coverages. With certain coverages, your cost is based on the amount of coverage you select and your age. With other coverages, your cost is a flat-per-$1,000-coverage rate. Therefore, it is important that you know how the Program works. Be sure to read all of the benefits information available, and ask questions so that you can make coverage decisions that are best for you and your family.

This information, along with the certificates of coverage, is your Summary Plan Description (SPD) and explains your life and accident coverages as of January 1, 2006 (unless noted otherwise). The information in the SPD is intended to serve as a summary of the Life and Accident Insurance Program. Because insurance coverage is provided through a group insurance contract with the claims administrator, the claims administrator has developed certificates of coverage to describe the coverage available to you under the Program. These certificates of coverage are included with this content.

This SPD, along with the certificates of coverage, explains your basic employee life insurance, employee supplemental life insurance, spouse life insurance, and child life insurance. It also explains voluntary accidental death and dismemberment insurance coverage. It details who is eligible for coverage, how to enroll, when coverage begins and ends, the benefits provided, how to apply for a benefit, and your rights under the Program. Please read this information to familiarize yourself with the Program.

The Plan document for the Life and Accident Insurance Program is entitled the “R.R. Donnelley & Sons Company Group Benefits Plan” (“Group Benefits Plan”). It governs the Life and Accident Insurance Program and includes more details on how it operates.

Minnesota Life is the claims administrator for the Life and Accident Insurance Program.

You are eligible for coverage under the Program only if you are an employee of a participating employer or subsidiary as identified in the certificates of coverage. As you review the certificates of coverage, be sure to check the eligibility to determine if the Program’s coverage applies to you, your spouse, and/or your dependent child(ren). If you are an employee of an employer or subsidiary that does not participate in the Group Benefits Plan, you are not eligible for the benefits described in this SPD. If you have a question about the Program, contact the eligibility administrator.

This SPD and any supplemental information, including the certificates of coverage, attempt to be as complete, accurate, and up-to-date a description as possible of your coverage under the Program. However, this summary is based on the Plan document and insurance contracts. If there is any discrepancy between this SPD versus the life and accident insurance contract, the life and accident insurance contract always governs. You and your beneficiaries should not rely on any oral description of the Plan or benefits because the insurance contract always governs.

In addition, nothing in this SPD should be interpreted as an employment contract. This summary merely describes the coverages offered to eligible employees as of January 1, 2006. RR Donnelley reserves the right to change or terminate the Group Benefits Plan or Program, in whole or in part, at any time.
