Enrolling for Coverage
General Information
If you and your spouse/domestic partner are eligible for coverage, you can enroll yourself and you can enroll your spouse/domestic partner for coverage under the Program. If you are eligible for coverage under the Program, an Enrollment Worksheet will be automatically sent to you after you terminate employment.
