Enrolling for Coverage
Eligible Surviving Spouse/Domestic Partner’s Enrollment
If You or Your Spouse/Domestic Partner Is Receiving Treatment When Coverage Begins
If you or your spouse/domestic partner already is in treatment under the Active Program at the time your coverage under the Program begins, the following procedures apply for all benefits – both medical, including prescription drug, and mental health and substance abuse care.
Inpatient Care: If you or your spouse/domestic partner is admitted to the hospital and is covered by the Active Program before your coverage under the Program takes effect and you or your spouse/domestic partner is still in the hospital on the coverage effective date of the Program, you or your spouse/domestic partner will continue to be covered under the Active Program for the hospital stay. Active Program coverage for you or your spouse/domestic partner for the hospital stay continues until you or your spouse/domestic partner is discharged. Your provider should call the claims administrator as soon as you or your spouse/domestic partner is discharged to discuss your transition plan.
Outpatient Care: If you or your spouse/domestic partner is receiving outpatient treatment, you or your spouse/domestic partner must have your transition plan preapproved by the appropriate claims administrator under the Program before your coverage under this Program takes effect. In all cases, the claims administrator(s) will make the final determinations about transition requests.
For more information about transition of benefits to your coverage under the Program, or to request any necessary forms, contact the appropriate claims administrator, listed in the “Administrative and Contact Information” section of this SPD.
