How to File a Claim
General Information
In some situations, you or your spouse/domestic partner may need to submit a completed claim form before the Program pays benefits. In other situations, you or your spouse/domestic partner does not need to file a claim.
Claims for benefits (including prescription drugs) must be submitted within one year of the date of service. Claims submitted after the deadline will not be reimbursed. Retirees and their spouses/domestic partners in the Moore pre-1979 and Moore 1979-1986 retiree groups should refer to the section “Special Rules for Certain Participants” for information on filing a claim.
