Enrolling for Coverage
General Information
If you and your spouse are eligible for coverage, you can enroll yourself and you can enroll your spouse for coverage under the Program. If you are eligible for coverage under the Program, an Enrollment Worksheet will be automatically sent to you after you terminate employment.
