Program Premium Cost
Determining an Annual Premium for You and Your Spouse
If You Are on an Authorized Leave of Absence
If you are an employee on an authorized leave of absence as determined under the provisions of your employer’s policies, you may be eligible for an annual subsidy cap amount if you meet the eligibility requirements for the annual subsidy cap amount as outlined above. For this purpose, service earned while on an authorized leave of absence from a benefits-eligible position will qualify as continuous service under the eligibility requirements for the annual subsidy cap amount of 10 or more years of continuous service.
