How to File a Claim
General Information
In some situations, you or your spouse may need to submit a completed claim form before the Program pays benefits. In other situations, you or your spouse does not need to file a claim.
Claims for benefits (including prescription drugs) must be submitted within one year of the date of service. Claims submitted after the deadline will not be reimbursed.
