Review of Initial Benefit Denial

Procedure for Filing an Appeal of a Denial

Any appeal of a denial must be delivered to the claims administrator for appeals within 60 days after receipt of the denial notice. Failure to appeal within the 60-day period will be considered a failure to exhaust all administrative remedies under the Plan and will result in your inability to bring a legal action to recover a benefit under the Plan (see the “Legal Action” section). The appeal must be in writing and delivered using one of the methods described above under the “Claims and Appeals Procedures – Procedure for Filing a Claim” section. The appeal must be delivered to the claims administrator for appeals at the address listed in the “Administrative and Contact Information” section of this SPD.
